ONLINE TERMS AND CONDITIONS FOR REFERRAL AGREEMENT
This Agreement is entered into between A Place for Mom (“APFM”) and the company on the Referral Program
Order (“You” or “Customer”). The Effective Date of your Agreement is the date of signing, except for certain Sections, which will be effective January 1, 2020, as noted herein.
